Technical liaisons who would like to receive 9-1-1 modification or consolidation plan filing notifications, or who have had a technical liaison change, should complete the 9-1-1 Vendor Technical Liaison Form and submit to isp.911tech@illinois.gov
911 Surcharge Forms
New 9-1-1 carriers are required to submit a Carrier Contact Form along with a current IRS form W-9. Please complete the form and submit to isp.911admin@illinois.gov
